Sample submission forms let customers or staff submit sample information through a guided form. Configure them under Admin → Forms (/admin/forms).
Permission required
You need sample.update to manage forms (same permission gate as the Forms admin page).
Open Admin → Forms
- Open the main menu.
- Go to Admin → Forms.
Manage forms
The table lists each form's label, status, and usage. You can:
- Create a new form
- Copy an existing form as a starting point
- Rotate or revoke form keys for security
- Use the copy icon on a row to copy that form's public submission link
- Show or hide revoked forms with the checkbox filter
Share a submission link (tenant operators)
After a form is configured, share it without opening Admin:
- Open Samples from the bottom navigation bar.
- Open the page ⋮ menu.
- Choose Copy Sample Submission Link to copy the public URL to the clipboard (uses your tenant's default sample form).
- Share the link by email or your usual channels. Recipients complete the guided form in their browser.
From the same menu, choose Upload Sample Submission Form to upload a completed PDF when your workflow uses paper forms.

Best practices
- Revoke forms that are no longer in use.
- Use clear labels so your team can identify each form in the list.
- Test a new form before sharing its link externally.
